This data use agreement covers all Spire Health Tag users. Affirmative electronic acceptance of the SPIRE DATA USE AGREEMENT is required for participation in any research or studies. The SPIRE DATA USE AGREEMENT allows Spire to share a de-identified subset of this dataset with business associates or partner organizations. A limited deidentified data set is defined as health information that excludes certain direct identifiers but that may include city; state; zip code; elements of date; and other numbers, characteristics, or codes not listed as direct identifiers (below).

Acceptance of this agreement is strictly voluntary; no rights, interests, renumeration or reimbursement will transfer or be provided before, during or after participation in the pilot program. This agreement constitutes the entire agreement between the parties and supersedes and extinguishes all previous drafts, agreements, arrangements and understandings between them, whether written or oral, relating to this subject matter.
